first_imgApr 25, 2007 (CIDRAP News) – The US Department of Health and Human Services (HHS) recently unveiled its plan for developing and buying medical countermeasures against a range of biological, chemical, and other threats, with new anthrax and smallpox vaccines among the near-term priorities.The 21-page implementation plan, released Apr 20 on the HHS Web site and in the Federal Register, details how the agency will acquire countermeasures against 14 threats on its priority list, which include nine category A biological agents, two category B biological agents, typhus, certain volatile nerve agents, and radiological and nuclear agents.HHS Secretary Mike Leavitt said in an Apr 18 press release that since the Sep 11 terrorist attacks the United States has made significant progress in securing medical countermeasures against a number of threats, but much more work remains.”This plan lays out our path forward in the coming years and will take advantage of our new authorities under the Pandemic and All-Hazards Preparedness Act to move forward with Project BioShield,” he said.Project BioShield, a $5.6 billion program, was established in 2004 to speed the development of medical treatments for the effects of biological and other unconventional weapons. But major drug companies showed little interest in the program. In passing the All-Hazards Preparedness Act last December, Congress tried to revitalize the program by authorizing partial payments to companies working under BioShield contracts before final delivery of their products.The HHS plan identifies acquisitions the agency will make under the remaining 5 years of Project BioShield, a 10-year program, as well as those the agency will pursue through fiscal year 2023.The report says the maturity of the product development pipeline and estimated costs guided HHS decisions and placement of items on near-term, mid-term, and long-term development lists. The agency focused on the countermeasures that offer the greatest opportunity to improve emergency preparedness across a range of threats.Priority is placed on countermeasures that involve “post-event prophylaxis” or postexposure treatment, the HHS report said. Though single treatments are appropriate for some of the biological threats—a “one bug, one drug” approach—HHS aid it wants to focus its efforts to more efficiently address groups of threats, using tools such as broad-spectrum antibiotics and broad-spectrum antivirals.The centerpiece of the HHS report is two tables: one that plots the priority countermeasures for each threat and another that lists the proposed funding sources for each countermeasure.Diseases listed among the priority threats include anthrax, botulism, glanders, meliodosis, Ebola and Marburg hemorrhagic fevers, tularemia, Argentine hemorrhagic fever, typhus, smallpox, and plague.Countermeasures that HHS would like to acquire in the near term, meaning in fiscal years 2007 and 2008, include broad-spectrum antibiotics and anthrax and smallpox vaccines. The report notes that HHS recently canceled a contract for a recombinant anthrax vaccine (a reference to a vaccine that was under development by VaxGen Inc.), but the agency “remains committed to acquiring next-generation anthrax vaccines.”HHS is also working to acquire a smallpox vaccine that would be safe for immunocompromised people, the report notes. The agency “is well advanced in the pre-award stage” of a program to buy one such vaccine, called modified vaccinia Ankara, it says.The list of products for mid-term acquisition—in fiscal years 2009 through 2013—includes broad-spectrum antibiotics, diagnostic tests for all biological agents, an anthrax antitoxin, filovirus countermeasures, and smallpox antivirals, as well as measures for diagnosing and treating radiological and nuclear agents and improving the distribution of nerve agent antidotes.Longer-term projects, beyond fiscal year 2013, would include broad-spectrum antivirals and a single, easy-to-use antidote effective against several volatile nerve agents.In its press release, HHS said it was seeking public comments on the implementation plan, which can be made through the Federal Register. first_imgJul 16, 2009 (CIDRAP News) – Citing the questionable usefulness of reporting pandemic H1N1 case counts and the burden it puts on countries experiencing widespread transmission, the World Health Organization (WHO) announced today it will no longer issue regular reports of confirmed global case totals.The WHO has issued 58 such reports since the start of the novel H1N1 outbreak, the last one on Jul 6.In a statement today, the WHO said countries with sustained community transmission are having an extremely difficult time confirming cases through laboratory testing. In addition, counting individual cases isn’t essential for monitoring the level or nature of risk posed by the virus or implementing response measures.Detecting and confirming all possible cases is highly resource-intensive, the WHO said. “In some countries, this strategy is absorbing most national laboratory and resource capacity, leaving little capacity for the monitoring and investigation of severe cases and other exceptional events.”For these reasons, the WHO said it will no longer issue reports of confirmed cases. However, it said it will provide regular updates on the spread of pandemic flu in newly affected countries.The focus of surveillance activities in countries where the virus is already established will shift to existing systems for monitoring seasonal flu, the WHO said. Countries are no longer required to submit regular reports of individual confirmed cases and deaths to the WHO.Monitoring for unusual events such as clusters of severe or fatal cases or changes in clinical patterns is important and should continue, the agency said. It added that countries should maintain vigilance for changes in transmission patterns, such as rising rates of school or work absenteeism, and also surges in emergency department visits, which could foreshadow increasing numbers of severe cases.Keeping close track of changes in the pandemic virus is also essential for case management and vaccine development, the WHO said. It recommends that even countries with limited lab capacity to follow up initial viological assessment by testing at least 10 samples a week.CDC says change is no surpriseTom Skinner, a spokesman for the US Centers for Disease Control and Prevention (CDC), said the change in the WHO’s case-reporting policy isn’t unexpected, because the WHO and CDC have been emphasizing over the past several weeks that the number of lab-confirmed cases is just the tip of the iceberg of the true number of people who are or were sick with the novel H1N1 virus.Specific case counts were once needed to help characterize the early spread of the disease, he said. Now that the virus is widespread and poised for a potential surge in the fall, “specific case counts are no longer needed, and since they don’t represent the true picture of the situation, they are not necessary,” Skinner said.The CDC will likely make a similar move to downplay the number of confirmed cases, but it will maintain, if not expand, surveillance to gauge the health impact of the pandemic and the severity of the illness, he said.Michael T. Osterholm, PhD, MPH, director of the University of Minnesota Center for Infectious Disease Research and Policy, publisher of CIDRAP News, said he fully supports the WHO’s policy change. The media and other groups have made too much of the case numbers, which grossly underestimate the illness burden, he said.”This a good thing, but it’s only the beginning of what needs to be done,” Osterholm said, adding that he hopes the WHO will introduce other comprehensive surveillance measures to more accurately gauge pandemic morbidity and mortality.Case counts give wrong impressionPeter Sandman, PhD, a New Jersey-based consultant and risk-communication expert, told CIDRAP News that early on in the novel H1N1 outbreak the WHO had a good technical reason to urge countries to track the number of confirmed cases—to help assess community transmission—but the usefulness of the numbers quickly waned.”The big problem is that almost everyone has used the number of confirmed cases as if it were the true number of cases,” he said. “Everyone was pouncing on those numbers.” Sandman said even CIDRAP News has made this mistake, repeatedly reporting the number of confirmed cases as if the number meant anything other than how many confirmatory tests were being done.The downside of the focus on the confirmed case numbers has been that the public perceives the pandemic as less pervasive than experts know it to be, Sandman said. When a public health official makes a reasonable extrapolation of the burden of disease, the number of confirmed cases seems puny by comparison. “Reputable experts are dismissed as nuts,” he added.Sandman said another problem is that an emphasis on the number of confirmed cases, along with the number of deaths, gives the public—and even some government and public health officials—an inflated impression of the case fatality rate.”The pandemic H1N1 virus could get more deadly at any time,” Sandman said. “But if you compare the number of pandemic deaths in the US to the CDC estimates of how many people have already had the disease, it calculates out that the pandemic is less deadly than seasonal flu so far.”However, most people don’t do that calculation, he said. “People compare the number of deaths to the number of confirmed cases, and that makes the pandemic look much more deadly, because all the cases that never got confirmatory testing are missing from the denominator.”See also:Jul 16 WHO statementlast_img read more

first_imgNov 3, 2009 (CIDRAP News) – The World Health Organization (WHO) said its team arrived in the Ukraine last night and met with the country’s health ministry today to discuss a dramatic spike in flulike illnesses and deaths, particularly in the western part of the country.The Ukrainian health ministry has recorded more than 250,000 flulike illnesses, of which 235 patients needed treatment in intensive care units, the WHO said in a statement today. As of yesterday, 70 deaths had been reported.The high level of illnesses activity and severe cases, combined with reports of hemorrhagic pneumonia cases, have raised some speculation that the virus might be behaving differently in the Ukraine, but further testing is just getting under way.Laboratory testing at labs in the Ukraine have confirmed the pandemic H1N1 virus in samples taken from patients in two regions, the WHO said, adding that because the pandemic virus has become dominant throughout the world, most of the Ukrainian respiratory illnesses are probably caused by the novel H1N1 strain.Though the highest illness rates have been seen in the western part of the country, illness activity is increasing rapidly in Kiev, the WHO reported.The WHO’s field investigation team will start in the Lviv region, where the number of cases of severe respiratory illness has been especially high.
